All Wellness plans are on a 12-month term. You may cancel your plan at any time with no cancellation fees if you haven’t used any of the Wellness benefits in your plan.
If you have used any of the Wellness benefits and would like to cancel before the end of the 12-month term, there will be a cancellation fee. The cancellation fee will be equal to the lesser of your total claim amounts/appointment costs and your remaining plan payments.
